The Health Division said that patients who were entitled to free medicines from the government, and who normally collected their medicines from Mosta Health Centre, or from the polyclinics of Bugibba, Gharghur, Mellieha, Mgarr, Naxxar, Qawra and St Paul’s Bay, should collect their registration form for the Pharmacy of Your Choice Scheme. This could be done from Mosta Health Centre or from the pharmacies which have joined the scheme.
Only those patients who had the yellow card or were diabetic should register with the scheme by 13 October.
Patients should take with them, when registering, their identity card, all the cards relating to diseases, and the white cards entitling them to free medicines.
The registration form must first be filled in by the pharmacist at the pharmacy chosen for the scheme, then patients should go to their doctor to have the second part filled in and then the form must be taken back to the pharmacy they would have chosen to register with.
Only the registration form provided by the Health Division can be used for registration purposes.
